Add a project task comment

Adds a comment, or a reply when parent_comment_id is given (the parent must be a live comment on the same task). note is BlockNote JSON, exactly what the web composer stores; markdown is derived server-side. Publishes the same TASK_COMMENT_ADDED event as the web, so assignee and @mention notifications fire. Only internal users may comment.
